Wellness Screener and Opt Out Form

In an effort to be more proactive in supporting students’ mental health, a wellness screener will be given on November 9 during Targeted Support at Da Vinci Science HS to identify students who may need support. Results from the screener will be used by the counselors to offer students interventions such as coping skills groups, counselor check ins, and referrals for therapy. Scores will not be shared with students or parents/guardians. Parent/guardian consent is required if a student is referred to receive counseling services delivered by Pepperdine and Stepping Stones therapists. If you would like your student to OPT-OUT and NOT PARTICIPATE in the screener, please fill out this Opt-Out Form by Thursday, November 5th.

Here are some resources from our partners, South Bay Families Connected; free parenting resources, videos and education events that cover a wide array of youth wellness topics. We encourage you to check-out their current SBFC Parent Newsletter and to sign-up to receive this free and informative publication. You are also invited to join their free monthly parent support group, The Families Connected Parent Chat. This month’s featured topic pages include College Admissions Stress, Mental Health and Substance Use Prevention
